On-Sale Intoxicating Liquor

• Business Application -complete form
• Alcohol and Gambling Enforcement On-Sale Certification Application
• Background Authorization form for each officer, owner and local manager
• Background and Licensing Fees
  • On-Sale Intoxicating Liquor Investigation Fee $500.00
  • Liquor License Yearly Fee- $3,750 (prorated)
  • Sunday On-Sale -$200.00
  • Cabaret-$375.00
  • Mechanical Amusement Device $20/site $20/machine

• Certificate of insurance showing liquor liability coverage.  Must coincide with licensing period of January 1 to December 31 each year*

*Certificate of insurance for liquor liability with the following minimum limits: $50,000 because of bodily injury to any one person in any one occurrence, and, subject to the limit for one person, in the amount of $100,000 because of bodily injury to two or more persons in any one occurrence, and in the amount of $10,000 because of injury to or destruction of property of others in any one occurrence.  $50,000 for loss of means of support of any one person in any one occurrence, and, subject to the limit for one person, $100,000 for loss of means of support of two or more persons in any one occurrence. The certificate of insurance must provide that ten days written notice will be given to the City should the liability insurance be discontinued for any reason. THE DATES OF COVERAGE ON YOUR CERTIFICATE OF INSURANCE NEED TO BE THE EXACT DATES THE CITY LICENSE IS ISSUED FOR. YOUR CERTIFICATE OF INSURANCE MUST STATE EFFECTIVE DATES 1/1/20 THROUGH 12/31/20 or THE POLICY MUST READ CONTINUOUS UNTIL CANCELLED.
